Medical cannabis and CNESST: reimbursement steps

CNESST provides a reimbursement mechanism for cannabis for medical purposes in certain workplace-injury files. Coverage is not automatic: the treatment must be accepted within the file and meet CNESST's medical, administrative and billing requirements.

The first condition: an eligible file

The request must be part of a recognized CNESST claim and relate to treatment of the workplace injury. A licensed practitioner must assess the situation and, if medical cannabis is appropriate, issue the required medical document. The clinical decision and the coverage decision remain separate.

What form 3300 (2026-02) contains

The 2026-02 edition of CNESST form 3300 is used only for billing. It requests the claim number, health care practitioner information, authorized daily amount, medical registration, supplier information, products, concentrations and dried-cannabis equivalents.

Rules in force since December 8, 2025

Sections 69 to 71 of the regulation in force provide that CNESST covers the actual cost of medical cannabis up to the equivalent of 3 grams of dried cannabis per day when it is prescribed by the health professional in charge of the worker, relates to the workplace injury, and is consumed in an ingested or transdermal form. An inhaled form may exceptionally be covered when that professional justifies it because of the worker's condition. CNESST remains responsible for applying these criteria to the file.

Direct billing and delivery

Under the regulation and official form, the supplier sends the invoice directly to CNESST and cannot claim that amount from the worker for the eligible service. CNESST covers delivery costs up to $20 per delivery. The supplier must confirm the file, products and quantities are eligible before the order.

Files already covered before December 8, 2025

A transitional provision protects certain files: when CNESST had accepted the cost of medical cannabis before December 8, 2025, new claims related to the same injury are excluded from the new conditions in section 69 while the prescription from the professional in charge remains unchanged. CNESST must confirm whether this exception applies to a specific file.
